    has been written down in its place below - Diacetylmorphine  Hydrochloride was the early chemical name for HEROIN - we have a very COOL piece here !!!

    Great original 1914 Treasury Department issued order form for Opium or Coca Leaves, or Compounds, Manufactures, Salts, Derivatives or Preparations.
    This form is ornate with an opium poppy graphic on the top left corner.
    This is a signed original and dated 1919.
    This form is for an order of
    HEROIN HYDROCHLORIDE
    and
    MORPHINE SULFATE
    preparations, probably war effort related.
    Its a rare piece to get both naming conventions of the drug Heroin listed directly on one form !!!
    its in pretty good used condition, a few light bends, otherwise, condition is excellent.
    A perfect size for framing for that doctor or Pharmacist in your life, would make a great gift.
